Chronic Disease Management & Rehabilitation

Your Accredited Exercise Physiologist can prescribe a unique exercise program to assist you in managing your chronic health condition or musculoskeletal rehabilitation.

Under the Medicare Chronic Disease Management (CDM) scheme if you have a chronic medical condition and complex care needs (as determined by your GP) you may be eligible for ”UP TO five bulk billed visits” billed visits to an allied health practitioner. Some common conditions where a referral may benefit;

 

Diabetes

Our type II diabetes group program is fully Bulk Billed under Medicare for any person with type I or type II diabetes who has a valid referral from their GP.

The diabetes program is run by an Accredited Exercise Physiologist (AEP) and is designed to support the management of blood sugar levels with appropriate physical activity.

 

Low Back Pain

Non-specific lower back pain (NSLBP) is one of the most common reasons a patient visits their GP each year. Targeted exercise interventions aimed at improving postural imbalance and stability systems throughout the body can be implemented at any stage of life.

Our AEP has significant experience across clinical and non clinical settings with the management of a wide variety of spinal conditions.

Cardiovascular Disease

Maintaining an active lifestyle is fundamental in the prevention and management of cardiovascular disease and associated conditions including; high cholesterol, high blood pressure.

Our AEP can work alongside you and your GP to set appropriate goals around management of the above conditions and build confidence with future self management.

Arthritis / Osteoporosis

Physical activity, specifically mobility training and strength training under the guidance of an AEP can be immensely beneficial for the management of both inflammatory joint conditions (arthritis) and loss of overall bone mass (osteoporosis).
